- Check your plan securely. Enter your insurance details in Headway and review eligibility, deductible, copay or coinsurance, and the estimated cost for your visit.
- Choose an appointment. Select the appropriate service and available time after reviewing your coverage. Attend by secure video while physically in Florida.

Alexeis welcomes eligible Medicare adults after the specific plan is verified. Original Medicare and Medicare Advantage have different coverage and network rules.

Our 30-minute self-pay follow-up is currently $100, regularly $120. We will maintain the $100 rate for one year and notify patients at least six months in advance of any change to this rate. Read the rate promise.
Clinically appropriate forms or letters: $25 extra during a scheduled visit or $75 outside a visit.
